Support includes community living, day services, supported employment services, and many more. Learn more ONLINE INTAKE Services are uniquely tailored to each person and family and are designed to allow persons with disabilities to lead self-determined lives and be full participants in their communities. To check the status of an eligibility determination, click here or call 1-844-ASK-DSPD or 1-844-275-3773 to speak with an intake worker. If you cannot remember the name of your intake worker, we can usually look that up for you. Find out where I am on the waiting list? To find out where you are at on the Waiting List, please call us at 1-844-ASK-DSPD or 1-844-275-3773 and ask for your Intake Worker. Please contact us if something has changed regarding your need to receive services. Appeal a decision affecting my services? DSPD will send you a Notice of Agency Action and Hearing Rights Form any time a decision is made affecting your services. That form spells out your options for appealing a decision made by DSPD. Typically all decisions from DSPD, give people two options: (1) You may take advantage of the Division Resolution Process; or (2) You have a right to proceed directly to an Informal Hearing before the Department of Human Services. In some cases you may also have the right to proceed directly to a Formal Hearing before the Department of Health. Please contact DSPD immediately if you have misplaced or have not received a Notice of Agency Action and Hearing Rights Form in the mail, following a decision affecting your services. Services are provided to those with the most critical needs first when funding is available. DSPD receives funding through appropriations from the legislature. More often than not, the Division is not able to offer immediate services to eligible individuals so they are placed on the waiting list. Renewal is completed through Utah Division of Motor Vehicles in your area. You should request a copy of form TC-842 titled “Disabled Person and Physical Disabilities Certification.” Have your doctor sign the form and return it to Motor Vehicles. Utah State Developmental Center The Developmental Center (USDC) offers 24 hour supervised residential and day training/employment services as well as community outreach services. The USDC has the capacity to serve up to 265 individuals. People with disabilities receive a variety of professionally driven support depending on their individual needs.
